Pichardo made his Major League Baseball (MLB) debut in 1992 and played in the league until 2002. During his MLB career, Pichardo played for several teams including the Houston Astros, Kansas City Royals, Boston Red Sox, and St. Louis Cardinals. He was primarily used as a relief pitcher and finished his career with a respectable record of 37-32 with 20 saves and a 3.96 ERA.

Pichardo is remembered for his outstanding performance during the 1995 Major League Baseball All-Star Game. He pitched in the 8th inning and struck out Tony Gwynn, Gary Sheffield, and Mike Piazza, sealing the American League's victory over the National League. Pichardo retired from professional baseball in 2003 and returned to the Dominican Republic where he has been involved in several charitable activities, supporting education and poverty reduction.
