Smith was drafted by the Chicago Cubs in the second round of the 1975 Major League Baseball draft. He spent six seasons with the Cubs and became one of the most reliable closers in the league. In 1983, Smith joined the Boston Red Sox and continued to have success as a closer, breaking the team record for most saves in a single season. In subsequent years, he played for several other teams, including the St. Louis Cardinals and the New York Yankees. Over the course of his career, Smith recorded a total of 478 saves, which was a record at the time of his retirement in 1997.

In 2019, Smith was inducted into the National Baseball Hall of Fame. His accomplishments as a pitcher were recognized on a national level, and he earned a place among the baseball greats. Today, Smith remains involved in the sport, working as a special advisor for the San Francisco Giants. He is remembered as one of the most dominant closers in baseball history and a true inspiration to young players everywhere.
