McDowell was drafted by the New York Mets in the third round of the 1982 Major League Baseball draft. He made his MLB debut in 1985 and spent ten seasons as a relief pitcher for the Mets, Philadelphia Phillies, Los Angeles Dodgers, and Texas Rangers. Over his career, he appeared in 723 games and recorded 159 saves. McDowell was known for his versatility on the mound and his ability to pitch multiple innings in relief. He was also known for his sense of humor, often playing practical jokes on his teammates.

After retiring from playing in 1996, McDowell transitioned into coaching. He served as a pitching coach for the Los Angeles Dodgers, Baltimore Orioles, Atlanta Braves, and Miami Marlins. McDowell was known for his success in developing young pitchers, and many of his protégés went on to have successful careers in the major leagues. In 2017, McDowell joined the coaching staff of the Seattle Mariners, where he currently serves as the team's pitching coach.
