Lilly's professional baseball career began in 1999 when he made his debut with the Dodgers. He played for the Montreal Expos, New York Yankees, Toronto Blue Jays, Chicago Cubs, and Los Angeles Dodgers throughout his career, from 1999 to 2013. During these years, he was considered to be one of the most dominant left-handed pitchers in the league. He was a two-time All-Star, in 2004 and 2009. In 2007, he was awarded the National League Pitcher of the Month award for his performances in May, with a 2.11 ERA, striking out 42 batters in 38.1 innings of work.

Lilly retired in 2013 with a career record of 130-113 and a 4.14 ERA. He had pitched for over 1966 innings and struck out 1681 batters. Known as a smart pitcher with a great sense of humor and leadership abilities, he was admired by teammates, coaches, and fans.
