Sanderson made his major league debut with the Expos on July 11, 1978, pitching 3 2/3 innings of relief against the Houston Astros. He spent three seasons with the Expos before being traded to the Chicago Cubs in 1984. He had his best season in 1989, winning a career-high 16 games with a 3.94 ERA for the Cubs.

Sanderson played for six teams in his 19-year MLB career, including the Cubs, Oakland Athletics, New York Yankees, California Angels, San Francisco Giants, and the Cubs again. He finished his career with a record of 163–143, a 3.84 ERA, and 1,660 strikeouts. In retirement, Sanderson served as a color analyst for ESPN and the YES Network for the Yankees. He passed away on April 11, 2010, at the age of 53 due to complications from cancer.
