Hamilton made his major league debut in 1961 with the Washington Senators at the age of 26. He pitched for the Senators from 1961 through 1963 and was then traded to the New York Yankees in January 1964. He had his best season with the Yankees in 1964, where he posted a 9-3 record with an ERA of 2.97. Hamilton was known for his unique pitching style, which involved a tall and leggy windup followed by a low and sidearm delivery. He was also popularly known for his effective use of the eephus pitch, a high-arcing pitch thrown at a slow speed.

Hamilton retired from baseball in 1972 after playing for nine seasons in the major league. He ended his career with a record of 40-31 and an ERA of 3.11. Hamilton played for four different teams during his time in the major league, including the Washington Senators, New York Yankees, Chicago White Sox, and San Francisco Giants. After his baseball career, he worked as a plumbing contractor and also taught baseball in youth leagues. Hamilton passed away on September 29, 1997, at the age of 62 due to heart failure.
