After high school, he was drafted by the Kansas City Royals in the first round of the 2002 draft, as a sixth overall pick. He spent his first full season with Royals in 2004, where he finished 8-11 with a 3.97 ERA. However, Greinke experienced personal struggles, including anxiety and depression, which affected his game. The team experience a significant setback after the team failed to reach the playoffs, and he was placed on the bereavement list after his grandfather died.

Despite the personal problems, Greinke's baseball career started to flourish from 2008 onwards. In 2008, he earned 13 wins with an impressive ERA of 3.47, and he was named the American League Cy Young Award-winner—an accolade given to the best pitcher in the AL season. Greinke would go on to play for other teams such as the Los Angeles Dodgers and Arizona Diamondbacks, earning several accolades and titles, such as the 2015 National League Cy Young Award as a member of the Dodgers.
