After graduating, Smith began his coaching career as an assistant coach at the University of Kansas. His big break came in 1961 when he was hired as head coach of the men's basketball team at the University of North Carolina. Smith quickly established himself as one of the greatest coaches of all time, leading the Tar Heels to two national championships in 1982 and 1993, as well as 13 ACC championships. He also coached some of the game's greatest players, such as Michael Jordan, James Worthy, and Vince Carter, and was known for his innovative coaching techniques, including the use of the "four corners" offense.

Throughout his career, Smith was a mentor to many players and coaches, and he was known as much for his commitment to his players' personal development as he was for his coaching abilities. He was also an advocate for social justice and famously recruited the first African American scholarship athlete at UNC, Charlie Scott, in 1967. Smith retired in 1997, having won 879 games throughout his career, and was inducted into the Naismith Memorial Basketball Hall of Fame in 1983. He passed away on February 7, 2015, at the age of 83.
