In 1963, Conigliaro signed with the Boston Red Sox as an amateur free agent, and by the following year, made his major league debut at just 19 years old. Known for his powerful swing, Conigliaro quickly became a fan favorite, hitting 24 home runs and being named the American League's Rookie of the Year. Conigliaro continued to excel in his early years with the Red Sox, hitting 32 home runs in 1965 and earning his first All-Star selection.

However, Conigliaro's promising career was derailed in 1967 when he was hit in the face by a fastball while at bat. The injury was so severe that he was temporarily blinded and needed multiple surgeries. Despite this setback, Conigliaro was determined to return to the field and did so the following year. Although he continued to have successes on the field, including leading the league in home runs in 1970, Conigliaro was never the same player he was before the injury.
