MacDonald made his NHL debut in the 2006-2007 season when he played for the Red Wings. He served as a backup goalie for most of his career, but he did have some memorable moments as a starter. In 2009, he replaced the injured Rick DiPietro as the Islanders' starting goalie and led the team to a playoff spot. His outstanding performance earned him a contract extension and the starting job for the following season.

Despite bouncing around to different teams throughout his career, MacDonald was a reliable backup goalie and a respected veteran in the locker room. He retired in 2017 after playing for the Beijing-based Kunlun Red Star in the Kontinental Hockey League.
